Letter to The Young Turks: Proof that Marijuana Smoking Does Not Lead to Lung Cancer

Dear Young Turks,

My name is David Malmo-Levine and I write for Cannabis Culture online magazine.

Recently, on your show, Cenk said, “if you smoke it very consistently for a long long period of time you can get lung disease … and maybe even lung cancer, as you would from smoking cigarettes.”

The facts say the opposite of this:

From Erowid’s “Marijuana Myths”:

There are no epidemiological or aggregate clinical data suggesting that marijuana-only smokers develop lung cancer.

You’d think it would have been very big news in June 2005 when UCLA medical school professor Donald Tashkin reported that components of marijuana smoke – although they damage cells in respiratory tissue – somehow prevent them from becoming malignant. In other words, something in marijuana exerts an anti-cancer effect!

There has never been a documented case of lung cancer in a marijuana-only smoker, and recent studies show that pot use is not associated with any type of cancer.

I attended the 1st National Clinical Conference on Medical Marijuana in Iowa in 1998. Arguably the most learned man on the subject of the effects of smoke on the lungs is named Donald Tashkin, Professor of Medicine, Division of Pulmonary & Critical Care, at UCLA. He spoke for almost an hour at the Conference, showing slides of smoke-browned lungs. He showed how marijuana smoke seems to push pollutants towards the outer lining of the lungs.

His presentation did not portray marijuana smoke as harmless, but there was no solid evidence of disease associated with the browning of the lungs’ outer lining. Donald Tashkin has conducted the most extensive research involving the largest number of long-term marijuana-only smokers, as well as multi-drug smokers–marijuana, tobacco, cocaine, and other substances, for several decades. His critique of marijuana smoke was completely non-committal. (Considering the government is his largest source of funding, there is little doubt why his lecture at the first medical cannabis conference was short and unsupportive.)

When Dr. Tashkin finished his speech, he did not wait to answer questions. He grabbed his materials and bolted for the door. I raced after him and stopped him on the steps outside the conference hall. I showed him the passages I had written about his research in The New Prescription. I stood there and watched him read everything I had written about his work. He agreed that my synopsis was correct. Then I asked him the $64,000 question. I said, “Do you know of any cases of marijuana-only smokers who had lung cancer?” He said “Yes, there is one.” Then he smiled, explaining, “He was sixteen years old.” We both smiled, knowing a teenager could not possibly have sufficient exposure to marijuana smoke to cause lung cancer–his cancer was clearly due to some other cause.

The Single Most Inportant Fact: Not One Recorded Case of Lung Cancer Linked to Cannabis

We have interviewed Dr. Tashkin numerous times. In 1986 I asked him about an article he was preparing for submission to the New England Journal of Medicine, indicating that cannabis smoking caused as many or more pre-cancerous lesions as tobacco in “equal” ammounts.

Most people do not realize, nor are the media told, that a pre-cancerous lesion is any tissue abnormality; abrasion, eruption, or redness. Unlike the radioactive lesions caused by tobacco, the THC-related lesions contain no radioactivity.

We asked Tashkin how many people had gone on to get lung cancer in these studies — or any other studies on long-term smokers like Rastas, Coptics, etc.?

Dr. Tashkin, sitting his UCLA laboratory, looked at me and said, “Well, that’s the strange part. So far no one we’ve studied has gone on to get lung cancer.”

“Was this reported to the press in the article?”

“Well, it’s in the article,” Dr. Tashkin said in passing, “but no one in the press even asked. They just assumed the worst.” His answer to us was still that not one single case of lung cancer in someone who only smoked cannabis has ever been reported. It should be remembered that he and other doctors had predicted 20 years ago, their certainty that hundreds of thousands of marijuana smokers would by now (1997) have developed lung cancer.

I hope this evidence will compel Cenk to retract his statement about cannabis possibly causing lung cancer, or at least be factually correct about the issue in the future. If anything, the evidence shows that cannabis may protect people from lung cancer, not cause it.

There is a relation to potency with concentrates, but it’s not at all directly proportional, and the biggest variables have to do with things like tolerance and quality of material, various cannabinoid profiles. If you have a person with no real tolerance, than it might seem very potent, because they just smoked the equivalent of a whole ounce in a single drop.

    If you have someone that can smoke a whole ounce unphased, the concentration of what they smoked will not get them any higher than could the profile of cannabinoids that was extracted.

    The point is that the concentration doesn’t reach some magical purity where it becomes a dangerous new drug. The only actual dangers associated with budder are the methods used to make it, which you attempted to confuse in your futile effort to be right at all costs.

    I will say it one last time, not for you but others who would be fooled by you. Wine is a limited subset with a fixed intrinsic max potency.     You raised the topic yourself and hold the ability to do so at a time of your choosing, so there’s no rush. That action affects the shape of the traffic flow curve. True success would have been to convince him to post an edit for his whole audience, not merely the fraction of it that’s shared with yours to see comments. That equates to preaching to the choir.

    He probably doesn’t like being wrong anymore than you do, and it might take awhile for him to come to his senses. Maybe what was really required was a better way of reaching them.

He’s sympathetic to marijuana, but it’s unclear if he’s really in the fight for it, or just against the drug war. It’s actually not an insignificant distinction. That’s to say that he’s not necessarily invested in your truth, and may be more invested in trying to seem moderate.

    That tactic of attempting to seem moderate, objective, or reasonable, as you can see, leaves the window wide open for misinformation of the highest absurdity. It leaves common sense and strong opinion on the fence because “the research just isn’t all in yet”, “we can’t be quick to judgement”. It conflates the intent of keeping an open mind with having an empty one.

    This is where Branson comes in. He is of some persuasion in the debate at this point, unfortunately. Those of his kind and position tend to argue on behalf of legalization or decriminalization from the strict perspective of doing it to eradicate the black market, as though that is the only conceivable ill born of prohibition. Laughable. It tends to call the bluff of the opposition, but that’s only good for a superficial nod while ignoring their underlying motivations. Bit of a pyrrhic victory that.

    That also leaves them free to argue the absurd. When they say skunk is super dangerous … not like what we all smoked in the 60’s that didn’t kill anyone, and that it causes schizophrenia to “flare up”, like it’s a rash, and only if they have it anyway. Such broken logic could easily be shot to hell, but rather than debunk it, it’s entertained, if not reinforced. “If we legalize low potency weed… the dangerous super potency black market killer skunk will be eradicated”.

    So on one hand you have people of his kin arguing that idiocy into the ears of high level politicians, and on the other hand you have “regulate like wine”… where as I already explained, has an inbuilt and potentially hidden limit on potency.

    If I were branson, perhaps wanting to manufacture and sell weed, I wouldn’t be at all opposed to regulations that restricted quality to that of very low production standards, costs and effort. Then it’s simply a question of economies of scale.

    Is it an absolute fact that marijuana doesn’t cause lung cancer? No, it’s not, but the science thus far indicates that it is highly, highly, highly unlikely that it does.     The fact of the matter is that there have been no cases of a marijuana-only smoker going on to get lung cancer provided by the leading investigator into this area of science – Dr. Donald Tashkin – or anyone else. That’s extraordinary evidence. You would think, in a world of about 300 million people who smoke pot, about 30 million in the USA and about 3 million in Canada, there would be at least one case of a marijuana-only smoker going on to get lung cancer, if cannabis smoking caused cancer, there would be many examples handy, but there are zero examples. This stands in contrast to tobacco smoking:

    I’m not sure this was worthy of such a story. This guy doesn’t make a habit of being wrong or saying crazy shit. He’s got his head on right and he’s definitely making very relevant observations about things, the drug war included.

    He is not a scientific authority on the issue however and his observations are common sense based, as they pertain to the information available to him.

    He’s probably a healthy guy. If he’s ever used it at all, he probably did so infrequently, and speaks from that perspective that he can enjoy, where it’s just basic common sense not to over indulge in anything.

    So maybe before picking an E-Fight with him, try contacting him and having a discussion before turning it into a big story like he owes you an apology or something.

    If the absurd becomes ordinary, then an extra ordinary claim doesn’t actually require extra ordinary evidence, but rather ordinary evidence. The burden of proof ultimately resides with the absurd to prove their absurd claims not only true, but relevant.

    Thus, one may choose to personally err on the side of caution, but not dictate extra ordinary measures of caution unto others, for that becomes the absurd.

    Maybe the problem is your qualifier – “in excess” – indicates that your solution – to stop smoking entirely – is unnecessary, when smoking less cannabis (of stronger cannabis or hashish) is all that is required.

    Of course, there’s no evidence that smoking cannabis “in excess” will lead to cancer either so you should probably not attempt to foist stigma where none need exist, because unlike your concerns, stigma IS harmful … it sometimes leads to discrimination … and occasionally genocide.

    … but some people prefer the effects from smoking over vaporization and eating … and smoking is faster than eating so you can titrate/control your dose … and not everyone can afford to buy a vaporizer … and smoking a joint is less likely to be detected than any vaporizer – even the hand-held variety … and GOOD weed, well grown – or small amounts of hash – is not as hard on the luns as the shwag you’e obviously been smoking … and you didn’t bother to address the issues raised in my post above, so no, problem not solved.

    The study does not say whether it has identified the cannabis smoke or the heat from the smoke as the major factor causing the problems. Perhaps extracts through a triple-filter water pipe would cause less damage than just smoking fatties all day. As well, the study does not differentiate between organic cannabis and those grown in radioactive chemical fertilizers. The study does not differentiate between low-grade Mexican shwag and extra-sticky dank nugs, that you don’t have to smoke as much to get the same effect from.

    Furthermore, you can’t say that people shouldn’t use cannabis daily. Some conditions – such as cancer, aids, glaucoma, epilepsy, and MS, require daily use, and some of these “daily use” conditions, such as depression, fatigue, stress, insomnia, are suffered by people we consider “healthy” or “recreational users”.

    It does cause increased incidence of lung infections, meaning bacterial. It’s really not a good idea to use Cannabis daily. Maybe on weekends. You get tolerant to it within a few days of regular use anyway. Like all good things, moderation is the key. I find it impossible to quit permanently. I can go for a few weeks and then I fall into a state close to depression. I guess you could just call it a lack of enthusiasm for life. If I then use a small amount it snaps me out of the malaise. It has a lot better effects when used infrequently too. So IMO it’s not completely harmless but if used occasionally it has benefits which seem to outweigh the negatives. It’s probably the best antidepressant available, and it doesn’t make you go on a killing spree like pharmaceutical antidepressants. The following is a quote about possible side effects. I guess using it in edibles would preclude all or most of these effects. It just takes so darn long to kick in.
